Don’t Miss Your Shot: Vaccinationland Sweepstakes Rules

Entry requires providing your name, date of birth, email address, phone number, home address, and the name and location of the place that provided your COVID-19 vaccine. Entry also requires agreement with all official rules of the drawing and agreement to allow the Maine Department of Health and Human Services to verify your vaccination information if selected the winner. A notarized authorization will be required to review immunization records for the winner verification process.

Entry for children between the ages of 12 and 17 further requires a parent or guardian’s name, email address, and phone number.

If you do not have access to the internet, you can call Maine’s Community Vaccination line at 1-888-445-4111 to provide information to enter the drawing.

Acceptance of the prize constitutes agreement by the entrant to maintain confidentiality of their winning status until after the State’s public announcement on July 4, 2021. Disclosure by the winning entrant to third parties prior to the public announcement may constitute forfeiture of the winnings.

Eligibility and Verification

Individuals are eligible to enter the Don’t Miss Your Shot: Vaccinationland Sweepstakes if they are:

  • A legal resident of the U.S. and a resident of Maine;
  • At least 12 years of age (for minors, a parent or guardian must complete the registration);
  • Not an employee of the Office of the Maine Governor, appointed State official, State employee involved in the drawing, or a household member of such employees; and
  • Have received at least one dose of Pfizer or Moderna COVID-19 vaccine or a single dose of Johnson & Johnson COVID-19 vaccine in Maine between December 15, 2020, and June 30, 2021.

General Rules

Entrants age 12 and over will be eligible for a random drawing for one (1) cash prize drawing. The State of Maine will verify all eligibility criteria prior to awarding any funds. Individuals who cannot furnish verification of eligibility as determined solely by State of Maine will be not be awarded any prize funds.

Each registrant must have a U.S. CDC Vaccination Card to claim the prize. If lost or unable to locate it, individuals should contact the provider where they received their vaccination or e-mail the Maine CDC at Registrants should have a U.S. CDC Vaccination card ready for July 1st.

It is the responsibility of each registrant to be available and respond to any outreach made by the Maine Department of Health Human Services following the July 1st drawing. Selected winners will have only hours to respond before the Department moves to an alternate selected registrant. Selected registrants may be required to travel to one of 16 regional DHHS offices to provide in-person proof of eligibility and complete paperwork.

The potential winner of the drawing will be randomly selected from among all eligible entries received. The selection process will mirror that used by the Maine State Lottery. Potential winners are subject to verification of eligibility and compliance with the Official Rules. Upon the random selection of an entrant, verification of eligibility, and confirmation of understanding of the Rules, the entrant will be declared the official winner of the drawing and will be announced by name and town of residence on July 4, 2021.

Eligible Mainers can submit only one (1) entry for the drawing. Prior to the drawing, all duplicate entries will be removed. Individuals who submit multiple entries will be disqualified.

All entries received by the deadline will be included in the drawing for the Don’t Miss Your Shot: Vaccinationland Sweepstakes promotion.

Alternate Winners

If a potential winner cannot be verified, violates the Official Rules, refuses to accept the prize, cannot be located, or is otherwise deemed ineligible at the sole discretion of the State of Maine, an alternate potential winner shall be drawn and declared the official winner upon verification.

The promptness and degree of reasonableness of efforts to locate a potential winner or any decision to declare a potential winner ineligible under these Official Rules shall be at the sole discretion of the State of Maine.

Alternates will be used in the order in which they were randomly selected as vacated spots arise.

Potential Winners

Any information entered for or by a minor child or adult under a guardianship must be validated by a parent or legal guardian prior to award of a prize. Any entrant subject to this condition must include the name, address, phone number and email address of the parent or legal guardian.

The State of Maine shall require presentation of information for verification prior to the award of any prize.

By entering the promotion, all entrants authorize the State of Maine to access vaccination records to verify that a COVID-19 vaccination has been received.

Other Conditions

Acceptance of the prize constitutes express permission for the State of Maine and its agencies as set forth in the Release and Verification to use the prize winner’s name and/or likeness worldwide, in perpetuity, for purposes of advertising and trade without further compensation, notification, or permission unless otherwise prohibited by law, in any and all media now known or hereafter devised.

Acceptance of the prize constitutes agreement to hold the State of Maine, any of its employees, affiliates, subsidiaries, contractors, assignees, and agents absolutely harmless against any and all claims and/or liability arising out of the ownership, acceptance, transference, and/or use of a prize. The prize winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in the Don’t Miss Your Shot: Vaccinationland Sweepstakes or use or redemption of any prize.

The State of Maine makes no warranty, guarantee, or representation of any kind concerning any prize and hereby disclaims any implied warranty.

By participating in the Don’t Miss Your Shot: Vaccinationland Sweepstakes, all entrants agree to be bound by the Official Rules.

These Official Rules may be amended or revised at any time at the sole discretion of the State of Maine.

The State of Maine reserves the right, at its discretion, to change the prize(s) to be awarded as part of this Promotion or end the Promotion prior to the completion of all scheduled drawing. Drawing date subject to change at the sole discretion of the State of Maine.

The State of Maine and any of its employees, affiliates, subsidiaries, contractors, assignees or agents are not responsible for late, or incomplete or misdirected entries, or for incorrect or inaccurate information received, whether caused by the website, or any of the equipment or programming associated with or utilized in this promotion.
