Maine’s Don’t Miss Your Shot: Vaccinationland Sweepstakes provides a great incentive to Mainers to get their COVID-19 vaccine. The drawing also raises awareness of how Mainers can obtain an appointment for one of these safe and effective vaccines.
There is one drawing:
Mainers 12 years old and older who have received at least one dose of a Moderna or Pfizer COVID-19 vaccine, or the one-dose Johnson & Johnson vaccine, in Maine may enter to win one drawing totaling $1 for every Mainer vaccinated by July 4th , 2021.
Who is eligible to enter the drawing?
Residents of Maine who have received at least one dose of the Pfizer or Moderna COVID-19 vaccines or the single dose of the Johnson & Johnson COVID-19 vaccine in Maine are eligible. A Maine resident who received their first dose of Pfizer or Moderna vaccine in another state but receives their second dose in Maine through 11:59PM on June 30, 2021, is eligible. A Maine resident who received both doses of Pfizer or Moderna vaccine in another state is not eligible.
Who is not eligible to enter the drawing?
Non-residents of Maine, and employees of the Office of the Governor of Maine, appointed State officials, State employees involved in the drawing, and anyone living in same household of such employees, are not eligible to enter the drawing.
How do I enter Maine’s Don’t Miss Your Shot: Vaccinationland Sweepstakes?
Entry into the drawing must be completed by 11:59pm on June 30, 2021. Entry requires providing your name, date of birth, email address, phone number, home address and the name and location of the place that provided you with the vaccine. Entry also requires agreement with the Official Rules of the drawing and agreement to allow the Maine Department of Health and Human Services to verify your COVID-19 vaccination information.
Entry for children between the ages of 12 and 17 further requires a parent or guardian’s name, email address and phone number.
If you do not have access to the internet, you can call 1-888-445-4111 to provide information to enter the drawing.
Is a parent or legal guardian required to enter the drawing on behalf of their minor child?
Anyone age 12-17 may enter the drawing themselves; however, they must provide contact information for their parent or legal guardian, who must validate the information before the prize is awarded. The State of Maine will contact the parent or legal guardian of any 12-17-year-old who is chosen as a winner.
Can I still enter if I am a Maine resident who was vaccinated in another state?
Maine residents who received at least one COVID-19 vaccine dose in the State of Maine are eligible for entry. A Maine resident who received their first dose of Pfizer or Moderna vaccine in another state, but receives their second dose in Maine through 11:59PM on June, 30, 2021, is eligible. A Maine resident who received both doses of Pfizer or Moderna vaccine in another state is not eligible.
Do I have to be vaccinated before I submit my entry?
Yes. The deadline to get at least one dose of the Pfizer or Moderna COVID-19 vaccines or the one dose of the Johnson & Johnson COVID-19 vaccine is June 30, 2021 as is the deadline to submit your entry.
When is the deadline to enter the drawing?
Entry Start Date: Tuesday, June 15, 2021
Entry End Date: Wednesday, June 30, 2021 at 11:59:59 p.m. EDT
Drawing Date: Thursday, July 1, 2021
Announcement Date: Sunday, July 4, 2021
How will I know if I have been entered into the drawing?
An entry confirmation will be displayed immediately after successful entry. No other confirmation will be available.
How will I be notified if I win the prize?
If your entry is selected, you will be contacted via the contact information provided on your entry form.
Where and when will the winner be announced?
The winner will be announced publicly by Governor Mills on the afternoon of July 4, 2021 in a press release and on social media.
How will the Department of Health and Human Services verify if I have been vaccinated?
By submitting an entry, you agree to allow the Maine Department of Health and Human Services to access your vaccination records, which will require a notarized authorization. You may also be asked to provide your vaccination card for verification. The Maine Department of Health and Human Services will only require a notarized authorization if you are selected as the winner.
What is the prize for the Don’t Miss Your Shot: Vaccinationland Sweepstakes?
Mainers 12 years old and older who have received one dose of a Moderna or Pfizer COVID-19 vaccine, or the single -dose Johnson & Johnson vaccine, in Maine may enter to win one prize valued at $1 for every Mainer vaccinated by July 4, 2021.

What are the tax implications of winning the prize?
Federal and State taxes will be withheld at 24% and 5% respectively. It is important to understand that the taxes withheld, if any, may not cover your entire Federal or State tax obligations. The final tax amount owed will depend on your personal tax situation.
The official winners should consult an independent tax advisor regarding all applicable tax laws.
Can I enter the drawing more than one time?
No. Only one entry per person will be accepted. All duplicate entries will be removed prior to each drawing and people submitting multiple entries will be disqualified.
Is my information private?
The State of Maine will not sell any information provided on this website. However, anyone who signs up for the Don’t Miss Your Shot: Vaccinationland Sweepstakes consents to allowing the Maine Department of Health and Human Services to verify your vaccination status. If you win, to accept the prize you must consent to the Release and Verification, which authorizes the State of Maine and its agencies to release the prize winner’s name and likeness.
Can I be disqualified?
Any violation of the Official Rules or inability to verify your vaccination status may disqualify you from winning the prize.

Who is paying for the prize?
The drawing will be funded from the CARES Act, Coronavirus Relief Fund allocated to the Maine Department of Health and Human Services for COVID-19 vaccination efforts.
When will the winner be notified?
Following the drawing on July 1st, the selected registrant will be contacted by a representative from the Maine Department of Health and Human Services (DHHS) by phone and e-mail. The selected registrant will have several hours to respond. If the selected registrant does not respond in a timely manner, the State of Maine will move to an alternate registrant. It is the responsibility of the registrant to respond to Maine DHHS’s outreach and provide all the requested information to verify eligibility. The selected registrant may need to travel to one of the 16 regional Maine DHHS offices to provide proof of residency, a copy of their vaccination card and complete paperwork. Please ensure that you have your U.S. CDC Vaccination card ready.
What if I don’t have my U.S. CDC Vaccination Card?
You should contact the provider where you received your vaccination to request a new U.S. CDC Vaccination Card or e-mail the Maine CDC at ImmunizeMe.DHHS@Maine.gov.
